**TL;DR.** Supporting statement evidence has to match the band. This page shows worked extracts at six bands (2, 4, 5, 6, 7, 8a) with commentary on why each scores. Use these as calibration — do not copy them, because panels recognise generic or recycled content quickly.
Band 2 extract (HCA, escalation)
*In my care home role I support eight residents with personal care and fluid balance. In February I noticed one resident had only taken 300ml across a six-hour shift and was more confused than her baseline. I raised it with the senior carer, who contacted the GP. The resident was treated for an early UTI. My senior said the early escalation had prevented a likely hospital admission.*
**Why it scores:** concrete situation, specific action (escalation), measurable outcome, attributable feedback. No inflation, no generic caring language.
Band 4 extract (Nursing Associate, pre-op assessment)
*I conducted 180 pre-op assessments in my first six months post-registration. In March I identified new-onset atrial fibrillation on pre-op ECG — outside NA scope — and escalated immediately to the surgical registrar and cardiology. Surgery was postponed, AF was treated, and the patient returned uncomplicated four weeks later.*
**Why it scores:** names NA scope explicitly, describes a scope-boundary decision correctly, quantifies volume and outcome.
Band 5 extract (Staff Nurse, deterioration)
*On a late shift a 74-year-old admitted for rule-out MI had NEWS2 of 2 at arrival. After 90 minutes BP dropped to 85/48 and NEWS2 rose to 7. I initiated SBAR to the SpR, put him on cardiac monitoring, gained IV access, and requested urgent troponin. He transferred to CCU within 25 minutes.*
**Why it scores:** specific clinical framework (NEWS2, SBAR), timeline precision, verifiable outcome.
Band 6 extract (Senior Staff Nurse, shift leadership)
*Mid-shift three pressures hit simultaneously: a NEWS2 deterioration, a family distressed about a DNACPR decision, and a new admission given the wrong oxygen regime. I coordinated the deterioration escalation, personally handled the family conversation, and corrected the oxygen prescription with documented handover and a datix. My line manager's feedback noted this as Band 6-level simultaneous leadership.*
**Why it scores:** names three simultaneous pressures, describes personal actions on each, references formal feedback.
Band 7 extract (Ward Manager, KPI recovery)
*SSI rate rose to 4.2% in Q2 2025, above trust threshold 2%. I led root cause analysis on the last 12 cases, redesigned dressing change ANTT protocol, commissioned training for every nurse, introduced weekly audit. Q4: SSI rate dropped to 1.6%. Approach adopted by sister ward.*
**Why it scores:** metric-driven, sustained intervention, cross-team adoption.
Band 8a extract (Matron, workforce recovery)
*Divisional safe staffing fill rate dropped to 78% against threshold 85%. I chaired weekly workforce meeting, commissioned skill mix review identifying three posts NAs could absorb, developed £180k establishment case approved by Director, led international recruitment for four senior posts. Fill rate recovered to 88%; sickness dropped 5.1% to 3.6%.*
